If the artist hears what is important to your sis, he/she may be able to come up with a personalized tat. My wife has an initial D for her first name wrapped in leafy ivy down the long side of the D. In the leaves, the veins are shaped into my initials. To anyone looking it won't be apparent.
It's a great tattoo but we paid $300 for it.
The short answer is find what is important to her, and figure out what image would she like, then get an artist's interpretation of it.
I never like tattoos but that one really sounds nice:o

http://www.tatusbykore.com/She's a very nice lady and does only custom work. My daughter also had a "flash" style tattoo in mind of a dragon for her 18th birthday. Kore came up with a different version which my daughter really liked - more than the original.
Add to that she has incense burning, lots of plants, completely sanitary and it really made a difference in her pain level during the process. If I ever decide to get another one I'd go back there.
