Found it a year ago. It's an excellent YTMND (though I haven't seen that many).

Simple. But poignant.

There are a few good YouTube vids out there on the same subject, but I've always loved the simplicity of this one, and the fact that you have to read it yourself accompanied with such melancholic music.
